Now I don't mean to sound cruel, but that sounds pretty creepy.
I thought of something else, imunizations, tetnus shots and that sort of thing.

but i have my own personal liter of blood frozen back home just for an emergency.
good luck getting that thawed and heated to the correct temperature...if you need a transfusion you are probably losing blood fast enough for that to take too long.

I'm really quite badly allergic to the tetanus vaccine. I was given one in my leg when i was about 6-ish and it swelled up like a balloon, so i'll have to find out what they use just in case.

I just had a thought about your frozen blood, not that it could be used for you and transfusions, but maybe as some sort of lure or even just drop it when you're being chased, perhaps they'll go for it.

Blood type
Mine are keep on my "dog tags", boots and sewn into my uniform as well. Medical records would be kept by the medics as well as the MD at the Higher Echelon.

lamination paper
you could use "lamination paper" so that it will not fall apart and keep a un laminated one at base to be updated when needed
